This document applies to both adult participants and parents or legal guardians registering minors.
PARTICIPATION CONSENT & RISK ACKNOWLEDGMENT
Natyanjali Productions Inc., operating as Natyanjali Dance Academy
1. Scope of Participation
Natyanjali Dance Academy provides instruction in Indian classical dance, including Bharatanatyam and Mohiniyattam.
Activities may include regular classes, complimentary trial classes, workshops, rehearsals, performances, and other academy-organized dance activities.
This document applies to participation in these activities, subject to any additional terms or consents required for specific events.
2. Acknowledgment of Risks
I understand that participation in dance activities involves physical movement and exertion.
Potential risks include slips, trips, falls, muscle strains, sprains, loss of balance, accidental contact with other participants, repetitive movement injuries, and accidents involving studio facilities, flooring, equipment, or performance environments.
I understand that injuries may occur despite reasonable precautions and supervision.
I acknowledge these risks and agree to follow the academy’s safety instructions.
3. Participant Health and Safety
I agree to inform Natyanjali Dance Academy of any medical conditions, injuries, allergies, or physical limitations that may affect safe participation in dance activities.
I understand that participants should not attend or participate in classes when experiencing an illness, injury, or other condition that could make participation unsafe.
Participants are expected to follow instructors’ directions, use appropriate dance attire, and participate only in movements and activities suited to their abilities and level of training.
Natyanjali Dance Academy reserves the right to modify, restrict, or temporarily suspend a participant’s involvement in activities when reasonably necessary to protect their safety or the safety of others.
4. Parent or Guardian Consent for Minors
If the participant is under 18 years of age, I confirm that I am their parent or legal guardian and have the authority to provide consent for their participation in activities organized by Natyanjali Productions Inc., operating as Natyanjali Dance Academy.
I voluntarily authorize my child to participate in dance classes, complimentary trial classes, rehearsals, workshops, performances, and other academy-organized activities.
I acknowledge that I have read and understood the risks associated with these activities, as outlined in Section 2 — Acknowledgment of Risks.
I agree to provide accurate and current information regarding my child’s medical conditions, allergies, injuries, or physical limitations that may affect safe participation. I understand that I am responsible for informing the academy of any significant changes to this information.
I agree to ensure that my child follows the academy’s safety instructions, classroom guidelines, and code of conduct. I also understand that Natyanjali Dance Academy may restrict or suspend participation when reasonably necessary for the safety of my child or others.
I understand that providing parental consent does not automatically waive or eliminate any legal rights belonging to my child under applicable Ontario law.
5. Emergency Medical Assistance
In the event of an injury, illness, or medical emergency during participation in activities organized by Natyanjali Productions Inc., operating as Natyanjali Dance Academy, I authorize the academy, its instructors, employees, and authorized representatives to take reasonable steps to protect the participant’s health and safety.
I understand and agree that:
a) Natyanjali Dance Academy may provide reasonable basic first aid within the training and abilities of its staff and instructors.
b) In the event of a serious injury or medical emergency, the academy may contact emergency medical services (911) and arrange ambulance transportation to an appropriate medical facility when reasonably necessary.
c) The academy will make reasonable efforts to contact the participant’s parent or legal guardian and designated emergency contact as soon as practicable.
d) If a parent, guardian, or emergency contact cannot be reached immediately, I authorize the academy to arrange emergency medical assistance without delaying necessary emergency services.
e) I understand that medical assessments, treatment decisions, and consent requirements remain subject to applicable Ontario laws and the professional judgment of qualified healthcare providers.
f) I acknowledge that any ambulance fees or medical expenses not covered by applicable provincial health insurance or other insurance may be my responsibility, to the extent permitted by law.
I confirm that the emergency contact and relevant medical information provided during registration are accurate to the best of my knowledge. I agree to notify Natyanjali Dance Academy of any significant changes to this information.
I understand that Natyanjali Dance Academy will handle medical and emergency contact information in accordance with its applicable privacy obligations and use it only for legitimate purposes related to participant safety and academy operations.
6. Participant Conduct and Property Damage
All participants enrolled in activities organized by Natyanjali Productions Inc., operating as Natyanjali Dance Academy, are expected to demonstrate respectful, responsible, and safe behaviour toward instructors, fellow students, staff, and other individuals.
I understand and agree to the following terms:
a) Compliance with Academy Rules
Participants must follow all reasonable instructions provided by instructors and authorized academy representatives, including safety procedures, classroom guidelines, and the academy’s code of conduct.
b) Respect for Others
Participants must treat fellow students, instructors, and staff with respect. Bullying, harassment, deliberate physical aggression, threatening behaviour, or conduct that endangers others will not be tolerated.
c) Responsibility for Personal Belongings
Participants are responsible for taking reasonable care of their personal belongings. Natyanjali Dance Academy is not responsible for loss, theft, or damage to personal property except where responsibility arises under applicable law.
d) Damage to Academy or Third-Party Property
Participants must exercise reasonable care when using academy facilities, rented studios, costumes, jewellery, equipment, and other property.
Where a participant intentionally, recklessly, or negligently causes property damage, Natyanjali Dance Academy may seek reimbursement for reasonable, documented losses from the person legally responsible, to the extent permitted by Ontario law.
For participants under 18, any financial responsibility of a parent or legal guardian will be determined according to applicable law and any valid, enforceable agreement. Parental status alone does not automatically establish responsibility for all damage caused by a child.
e) Injuries Caused to Other Participants
Participants must avoid behaviour that could reasonably endanger themselves or others.
If a participant causes injury to another individual through intentional misconduct, reckless behaviour, or negligence, legal responsibility will be determined according to the circumstances and applicable Ontario law.
Nothing in this agreement automatically transfers Natyanjali Dance Academy’s own legal responsibilities to a participant or their parent or guardian.
f) Disciplinary Measures
Natyanjali Dance Academy reserves the right to issue warnings, require corrective action, temporarily suspend participation, or terminate enrollment where reasonably necessary because of serious or repeated violations of safety rules or the code of conduct.
Any disciplinary action will be administered fairly and consistently, taking into account the participant’s age, circumstances, and applicable legal obligations.
g) Reporting Incidents
Participants and parents or guardians are encouraged to promptly report injuries, unsafe conditions, damage to property, or concerning behaviour to an academy instructor or authorized representative.
Natyanjali Dance Academy will take reasonable steps to review reported incidents and address identified safety concerns.
7. Liability, Release and Indemnification
a) Acknowledgment and Voluntary Assumption of Risks
I acknowledge that participation in Bharatanatyam, Mohiniyattam, and other dance-related activities involves inherent risks, including physical injury, accidents, and potential damage to personal property.
I understand the nature of these activities and voluntarily agree to participate, or authorize my child to participate, with knowledge of the associated risks.
b) Release of Liability — Adult Participants
For participants aged 18 years or older:
To the fullest extent permitted by applicable law, I voluntarily release and discharge Natyanjali Productions Inc., operating as Natyanjali Dance Academy, and its directors, officers, employees, instructors, independent contractors, volunteers, and authorized representatives (collectively, the “Released Parties”) from claims, demands, actions, and liabilities arising from my participation in academy activities, including claims alleging negligence by the Released Parties.
I understand that this provision is intended to release the Released Parties from liability for their own negligence, subject to applicable law.
This release does not apply to liability that cannot legally be excluded or released.
c) Parent or Legal Guardian Consent — Minor Participants
Where the participant is under 18 years of age, I confirm that I am the participant’s parent or legal guardian and authorize their participation in academy activities.
I acknowledge and accept the inherent risks associated with these activities.
I understand that signing this agreement does not necessarily waive or extinguish any independent legal claim belonging to my child, including claims arising from negligence.
d) Participant Conduct and Responsibility
Participants are required to follow reasonable safety instructions and conduct themselves responsibly.
Where a participant causes injury to another individual or damage to academy or third-party property through intentional misconduct, reckless behaviour, or negligence, legal responsibility will be determined according to applicable law.
e) Indemnification — Adult Participants
For participants aged 18 years or older:
To the extent permitted by law, I agree to indemnify and hold harmless the Released Parties against reasonable losses, liabilities, damages, and legal expenses arising from third-party claims directly caused by my intentional misconduct, reckless behaviour, or negligent acts during academy activities.
This indemnification does not apply to losses caused by the negligence, misconduct, or breach of legal obligations of the Released Parties.
f) Parental Responsibility and Indemnification — Minor Participants
Where I am the parent or legal guardian of a minor participant, I agree to take reasonable steps to ensure that my child follows the academy’s safety rules, instructions, and conduct requirements.
To the extent legally enforceable, I agree to reimburse Natyanjali Dance Academy for reasonable, documented losses arising from property damage or injury to third parties directly caused by my own negligent supervision, my own wrongful conduct, or my child’s intentional or reckless misconduct, where a valid legal basis establishes my responsibility.
This provision does not automatically make me liable for every act of my child. It does not require me to indemnify the academy for its own negligence or waive my child’s independent legal rights.
g) Claims Involving Minor Participants
I acknowledge that claims involving injuries to minor participants are subject to applicable Ontario law.
Nothing in this agreement is intended to automatically eliminate a minor participant’s right to pursue a legally available claim.
Any proposed obligation requiring a parent or guardian to reimburse Natyanjali Dance Academy for a claim brought by their child, including associated legal expenses, must be separately assessed for validity and enforceability under Ontario law.
h) Limitations and Applicable Law
Nothing in this agreement is intended to exclude liability or waive legal rights where doing so is prohibited by law.
If any provision is determined to be invalid or unenforceable, the remaining provisions will continue to apply to the extent legally permitted.
This agreement is governed by the laws of Ontario and the applicable federal laws of Canada.

The acknowledgment and applicable agreements are intended to apply individually to each named participant.
A single electronic signature may be used to record consent for multiple minor participants where the signer has the legal authority to provide such consent.
